Looking at the design, just in pictures mind you, I am not surprised that accidental opening occurred.

It's a good looking piece of rigging.
What's the main use of the device? Climbing / slack tending? In which case you could become complacent and forget to inspect it regularly. You're using it hours at a time on every climb
Light rigging applications? Not as likely to fail there, I'd think. Only because you're breaking it down and re-setting it in most applications.
Nothing is fool proof. Just my 2 cents.
