It surely is not the perfect answer but I have never had it come completely off during a climb. I do however have a small tether on it in case it does.
I was just at a conference that had an SRT presentation. I had the RR the BDB the UNI and the HH all there for climbers to try. Those that climbed on my properly tuned HH instantly recognized its superior attributes. The HH is not only indestructible but is capable of superb performance.
Nice job! Have you tried that with a knee ascender also? By the way, I have used other newly developed multi-senders since that time and have not changed my opinion of the HH and the setup being used. It rocks!
Tim, that is a very clever setup. Be careful though, you have a lot going on with the way you have it. Have you tried the HH carabiner in the middle hole or the top hole and the bridge carabiner in the bottom hole?
If the tether doesn't capture the carabiner, when you pull up on the tether the carabiner will not release. You will most definitely feel that extra drag. Even with a pulley under the HH the carabiner will move up first when tending.

I use short pieces of bigshot tubing for trapping carabiners on many things. I have not found them to work for the HH carabiner. There is not much space left around the sides of the HH after the carabiner is installed. If you try to force too much material in that space things will not slide...
Sean, if you are using a pulley under the HH that is your slack tending point. For single line rope walking just clip a chest or shoulder harness into the bridge ring. Used with a foot and knee ascender this works great.

Yes you do loose some efficiency when using a revolver compared to a pulley, but it is a 3:1 haul back system so even at reduced efficiency it is still easy.
Shaun, I am using the red ISC fixed side plate pulley with a DMM bridge ring but I also have a Petzl fix pulley that I am pretty sure will fit without bending. You may notice that I cut my pulley because it is a pain to take on and off otherwise, but it does work.
