I wondered if there was an internal maintenance ladder, too. Maybe that's how the first guy got up there. Would guess they had a belay AND a lowering system.
 
I bet it was an exciting resue! Looks like a fun belay and rigging challenge.
 
There is like a step ladder along the rail on the outside (link 2 pic 1), maybe one step for 20" or 2feet. Not very practical for the customers.
The firefighters seems to came from the top of the mast ( pics 4 and 7). You can see there a steel platform just beside the step ladder's top and a man hole a little farther to the right. I bet there's an other ladder inside the mast, and probably an elevator to get to the machinery room on top (pic 9). But for the midway trapped visitors, I suppose that it was as much impractical to reach than the ground.
 
7-8 hours is a long time with no bathroom break or drink of water for most people. I wonder how the fire crew would not know ahead of time that their ladder truck would not reach.
