Lube the form with crisco or a fancy form release agent, order glass reinforced concrete with a medium slump of 4-6, I would go 6" thick, cut/saw in your control joints just after initial set, keep the slab wet for 28 day after pour to ensure maximum cure, if the pour is to encounter temps below...- flushcut
